New Authorizations (Initial Setup)
- Fill in your personal information.
- Check the box for New payment from account specified below.
- Fill in your Bank Name.
- Check the box that corresponds with the type of bank account the withdrawals will be made from. Be sure to attach proof of the account (a voided check for checking accounts or a blank deposit slip for savings accounts).
- Your Routing Number is the first 9 digits printed at the bottom of your checks or deposit slips. This is your bank’s electronic address.
- Your Account Number is the next set of numbers on the bottom of your check or deposit slip. Do not put in spaces or punctuation that is shown. If copying the information from a check, be careful not to include the set of numbers farthest to the right. These numbers correspond to your check number, and are only there to assist your bank with the checking process.
- Authorization Effective Date is the date on which you authorize us to activate your electronic giving information. Please note that this is not necessarily the date the first withdrawal will be made from your account. The first withdrawal will be made on the first collection date on or after the authorization effective date that corresponds with your payment schedule. In most cases, you can just put today’s date as the authorization effective date. The only time a later date is needed is if you wish to delay the start of your electronic giving.
- The Contribution Amount Worksheet is included to assist you with converting your weekly donation amount to an equivalent amount for monthly, quarterly, semi-annual or annual collection. Everyone should fill in line 1 and line 2. One additional line on the worksheet needs to be filled in if you are going to donate monthly, quarterly, or semi-annually.
- Check the box under Contribution Schedule that corresponds with how frequently you wish to contribute.
- The Contribution Amount is the amount that you wish to have withdrawn from your checking or savings account each time an electronic payment is processed. It is the amount you calculated in the Donation Amount Worksheet.
- Check the box under Collection Date indicating if the amount is to be withdrawn on the 1st or the 15th of the month.
- Sign and date the form.
- Attach a voided check or blank savings deposit slip to the form.
Change of Information
- Please keep in mind that a two-week written notice is needed for any changes.
- Fill in your personal information.
- Check the box for Change indicated below.
- Complete the section(s) of the form that corresponds with the information you wish to change. You do not need to fill out the entire form again. See the New Authorization instructions for detailed information on filling out a specific section.
- Sign and date the form.
Termination of Electronic Giving
- Please keep in mind that a two-week written notice of termination is needed.
- Fill in your personal information.
- Check the box for Discontinue electronic funds transfer from account specified below.
- Complete the Bank Account Information section. You do not need to fill out the entire form again. See the New Authorization instructions if detailed information on filling out the section is needed.
- Sign and date the form.
